Participants:
As the 2020 Asian Water Polo Championships, which should have been the Asian continental qualifiers for the Olympic tournament, could not be held due to the COVID-19 pandemic, it was decided that the results from the 2018 Asian Games applied.
- KAZ (placed second at the 2018 Asian Games)
As all the next placed nations (THA, INA, and HKG) delined the invitation UZB was invited, but they also withdrew shortly before the tournament.
NZL was selected as Oceanian representative, but also withdrew shortly before the tourament.
HUN and NED qualified for the 2020 Tokyo Olympics.
